Probable Alzheimer's disease

Elderly patient with advanced dementia and cognitive decline.


Case Discussion

Selected images from an MRI demonstrating very advanced atrophic change including:

  • the hippocampi (contrary to earlier reports they can be in FTD)
  • frontal and temporal lobes 

The differential is largely between Alzheimer's disease and frontotemporal dementia (FTD).

The temporal arophy is more prounced than fronal atrophy. This patient also presented with more cognitive & severe  memory  loss  than behavioural changes, which are common presenting syndrome in Pick's disease. In late stage, with inolvement of frontal lobes , it is difficult to differentiate one from other  & one may have to rely more on clinical presenation.
