Progression of MCA Infarct

Patient Data

Age: Adult

Initial CT of the brain (single slice) demonstrates loss of grey white matter differentiation in the left MCA terriotry. 


CT obtained sometime later demonstrates gyral and caudate head enhancement. 

Case Discussion

Without further information regarding time course or therapy it is difficult to definitely interpret these findings. If the second scan is a couple of weeks after the initial CT this would represent a similar appearance to fogging. If this however was seen early in the acute period it may represent luxury reperfusion seen when an occlusive clot is broken down (either spontaneously or pharmacologically).
