Progresssive iatrogenic pneumothorax from incorrectly connected Heimlich valve

Presentation

Follow up chest x-ray to assess pneumothorax.

Patient Data

Age: 60 years
Gender: Female

Iatrogenic pneumothorax from port placement.

Iatrogenic tension pneumothorax from wrong Heimlich valve connection.

Blue coloured end attaches to patient end.

Case Discussion

On physical exam hyperresonance noted on left lung fields when compared to right, with no lung sounds auscultated on left side when compared to right side.  

Patients pigtail was examined also at this time with the Heimlich valve noted to be placed backwards with the pigtail closed off to prevent air from escaping.  

Heimlich valve connected to atrium with 20 cm to wall suction with notable air bubbles present after connection.
