Properitoneal inguinal fat herniation

Case contributed by Dr Praveen Jha

Non-specific abdominal pain, with incidental inguinoscrotal swelling on right side. No hernia suggested by surgeon.

Patient Data

Age: 42
Gender: Male

The right inguinal canal is dilated with fat attenuation contents within and around the spermatic cord, extending down into the right scrotal sac. No bowel herniation.

Note: Technologist did not include scrotum in the scan, as he followed routine abdomen protocol and didn't notice the inguinal canals at the time of scanning, or considered it insignificant.

Case Discussion

Differential diagnosis:

  1. Properitoneal fat herniation in inguinal canal.
  2. Spermatic cord / inguinal canal lipoma

Provisional diagnosis of properitoneal fat herniation was considered, and being an incidental finding, no further intervention suggested at present.
