Bone pain and right lower limb paresthesia.
Patient Data
The prostatic gland is measuring about 35*36*40 mm and shows heterogeneous signal intensity.
Hyposignal tumoral mass lesions in T2 WI is present at prostatic gland peripheral zone more of obviously in the left side associated with a 30*35 mm infiltrative soft tissue tumoral mass with water restriction and invasion to left seminal vesicle, mesorectal fascia, and rectal wall as well as levator ani muscle.
There is also another large hyper signal mass with water restriction measuring about 70*85 mm at the right side of the pelvic cavity with pressure effect on the bladder and mesorectal fascia and extension into the right sciatic notch associated with probably involvement the sciatic nerve.
The signal intensity of the pelvic bone marrow was diffusely inhomogenous consistent with sclerotic metastasis.
Case Discussion
The recent patient previously was a known case of prostatic adenocarcinoma at TRUS biopsy a year ago.
Regarding the imaging in findings, prostatic adenocarcinoma with local invasion and right internal iliac chain huge conglomerated metastatic lymphadenopathies with extension into the sciatic notch is seen.`
