Prostate nodule - PI-RADS 2

Elevation of free PSA level

Patient Data

Age: 75 years
Gender: Male
MRI

Prostate size is enlarged measuring 34 x 46 x 51 mm, resulting in a calculated 41 mL volume.

There is a T2 hypointense nodule (12 x 10 mm) at the anteromedial aspect of the transition zone of the apex of the left prostate. It shows diffusion restriction on DWI and early washout on post-gadolinium images.

No periprostatic extension is found and the capsule is intact.

The central gland is enlarged due to BPH.

Case Discussion

The constellation of findings is unlikely to represent prostate cancer, as the nodule attains a PI-RADS score of 2.
