Age: 30 years
Gender: Male

There is a small well-defined midline cyst of the prostatic gland of low signal on T1WI and high signal on T2WI. It seems to communicate with the posterior urethra on sagittal T2 WI with beak-like towards the urethra on axial T2WI 

Case Discussion

A small midline prostatic cyst communicating with the posterior urethra is most consistent with a prostatic utricle cyst which is difficult or impossible to differentiate from a Müllerian duct cyst (midline prostatic cyst not communicating with the urethra)
