Pseudokidney sign of intusseception

Presentation

3 days of severe abdominal pain, coming in waves, lasting 5-10 minutes. Intussusception?

Patient Data

Age: 2 years
Gender: Male
ultrasound

The scan demonstrates pseudo-kidney sign of the bowel loops underneath the liver. The intussusception approximately measures 6 x 3 cm. It is situated in the right upper/mid quadrant.

Central hyperechoic mesenteric fat and enlarged mesenteric lymph nodes of around 11 mm.

The liver, both kidneys, gallbladder, and pancreas are normal.  No free fluid in the pelvis.

 

Case Discussion

Ultrasound findings are consistent with an ileocolic intussusception with enlarged mesenteric lymph nodes centrally.

The second image nicely illustrates the pseudokidney sign as it looks like 2 kidneys next to each other.

The transverse images equally well illustrate the doughnut sign.
