Age: Neonate
Gender: Female
x-ray

Nasogastric tube is in place with tip projected over the stomach.

There is a well defined oblique line along the left lateral chest. Some lung markings are seen beyond the superior margin and is most suggestive of a skin fold giving the impression of a pneumothorax i.e. pseudopneumothorax.

Case Discussion

Skin fold is a great mimicker for a pneumothorax particularly in the neonate and the elderly, where flaccid skin at the back is sharply outlined. Identifying the lung markings beyond the skin fold margin is helpful in clarifying the situation and avoiding unnecessary further imaging or intervention.
