Pulmonary arteriovenous malformation

Case contributed by Bassem Marghany
Diagnosis almost certain

Presentation

Chest pain and shortness of breath; Right lung nodule on chest X-ray.

Patient Data

Age: 45 years
Gender: Male

Well defined non-calcified nodule at the anterior segment of right upper lobe with a single feeding arterial branch derived from the right upper lobe pulmonary artery and a single draining vein seen merging with the right superior pulmonary vein (pulmonary AVM - simple type).

Incidentally aberrant right subclavian artery and enlarged thyroid gland.

Case Discussion

Pulmonary AVM is a common cause of solitary pulmonary nodule (coin lesion).

Multiple pulmonary AVMs should raise the possibility of hereditary hemorrhagic telangiectasia.

Contrast-enhanced CT is the diagnostic modality of choice.
