Pulmonary artery thromboembolism

Presentation

Dyspnea, chest discomfort since 2 days.

Patient Data

Age: 40 years
Gender: Male

A hypoattenuating non-enhancing intraluminal thrombus measuring 6.1x1.7cm in the right pulmonary artery extending from the bifurcation of the pulmonary trunk to the lower lobar segmental branches. The thrombus is resulting in subtotal ( > 80-85% ) luminal obstruction.

A soft tissue attenuation nodular lesion with cavitation is noted in the subpleural region of superior basal segment of right lower lobe representing pulmonary infarct with secondary infection.

The diameter of the main pulmonary trunk exceeds the diameter of the aortic arch.

Imaging features are suggestive of chronic pulmonary artery thromboembolism with pulmonary infarct and possible pulmonary artery hypertension.

Case Discussion

Pulmonary thromboembolism is potentially life-threatening however curable condition. Early diagnosis and treatment improve the outcome. Cross-sectional imaging is the mainstay of diagnosis.
