Pulmonary cryptococcosis

Presentation

Diabetic patient, with abscess in back.

Patient Data

Age: 40 years
Gender: Male

Multiple parenchymal nodules are seen scattered in bilateral lung fields with no zonal predilection. A nodule in right upper lobe shows cavitation within. Nodules are predominantly arranged in periphery. Abscesses (post-drainage on right side), are seen on posterior thoracic wall, involving trapezius, subscapularis, and latissimus dorsi muscles.

Case Discussion

Abscess drain aspirate gram stain showed gram +ve cocci, however culture resulted cryptococcosis. Characteristic pulmonary nodules, and diabetic status, should suggest pulmonary cryptococcosis, with dissemination.
