Breathlessness and pleuritic chest pain. The clinical diagnosis was of PE.

Case Discussion

The initial view of the CTPA doesn't reveal any large filling defects, but closer inspection reveals clot adherent to walls of both the left and right main pulmonary arteries before their bifurcation.
