Shortness of breath and chest pain on a background of antiphospholipid syndrome.

SPECT ventilation and perfusion scans of the lungs were performed. A large mismatched perfusion defect is seen in the anterior segment of right upper lobe. Small mismatched defects are also seen in the superior segment of the left lower lobe and lateral segment of the right middle lobe. No other mismatched defects noted.
Conclusion: The scan findings are highly suggestive of multiple pulmonary emboli.
Case Discussion
This patient has had multiple admissions with deep vein thrombosis and pulmonary embolism given her diagnosis of antiphospholipid syndrome.
