Pulmonary fibrosis - early

Case contributed by Dr Ian Bickle

Presentation

Progressive shortness of breath. Inspiratory fine crepitations on auscultation.

Patient Data

Age: 50 years
Gender: Male
X-ray

Diffuse fine increased interstitial markings in both lungs without a zonal predominance.

Lung volumes normal.

Heart size normal.

Case Discussion

This film demonstrates relatively early fibrosis (for a chest radiograph):

1. increased interstitial markings

2. A 'shaggy' or instinct outline to the heart (due to adjacent lung fibrosis)

3. at this stage the lung volumes are maintained.

The causes of lung fibrosis are multiple.

HRCT chest is typically performed to assess the extent of the disease in a more sensitive fashion.
