Back pain. History of sarcoidosis.
Patient Data



Lung fibrosis, more severe on right and particularly pronounced in right apex.
Hila pulled cranially, trachea pulled to right.
Bilateral hilar lymph nodes with peripheral calcification.
Right hemidiaphragm higher than the left, probably pulled by severe fibrosis.
Case Discussion
This case demonstrates end-stage (stage IV) pulmonary sarcoidosis.
There is bilateral pulmonary fibrosis. Hilar and mediastinal lymph nodes with eggshell calcification are seen; on the right, the lymph nodes appear to be located in the lung apex because the hilum is pulled up by the fibrotic lung.
