Presentation

Weight loss, fever, and productive cough with a history of pulmonary tuberculosis.

Patient Data

Age: 35 years
Gender: Male

Consolidation, large irregular margin cavities, bronchiectasis, and decreased volume in the right upper lobe with right side deviation of the upper mediastinum and multiple patchy and nodular consolidations with different sizes in lungs are seen which have a relatively ill-defined margin with the mostly peribronchial and peribronchiolar distribution. The peribronchial soft tissue thickening is also noted.

Case Discussion

The case illustrates the imaging features of both primary and post-primary pulmonary tuberculosis. The diagnosis was based on MDM discussion, strongly positive Mantoux tuberculin skin test, positive sputum culture, and positive PCR test for mycobacterium tuberculosis.
