Pyomyositis with thigh abscesses

Presentation

Known patient with IV drug use presented to ER by left thigh painful swelling , fever and limited movement.

Patient Data

Age: 30 years
Gender: Male

Extensive left thigh intermuscular infection with widespread heterogenous loculated collections and surrounding muscle edema involving mainly the anterior muscle groups. Mild focal femur shaft marrow edema with no cortical breach.

Preserved femoral vessels. However the distal segment of profunda femoral vessel appears ectatic with eccentric mural filling defect. 

Case Discussion

Extensive left thigh deep muscular infection with localized collections/abscesses and surrounding marked edema as well as inflammatory changes.

The collection was aspirated under US guidance and revealed it contained pus.
