Quadriceps fat pad impingement syndrome

Diagnosis almost certain

Presentation

Pain of the anterior aspect of the left knee. No history of trauma.

Patient Data

Age: 40 years
Gender: Male

The quadriceps fat pad is enlarged of high signal on PD fat sat and T2 best seen on axial PD fat sat with mass effect on the adjacent suprapatellar recess and quadriceps tendon.

Small linear hyperintensity of the posterior horn of the medial meniscus with no extension to the articular surface, grade 2a according to the MRI grading system for meniscal signal intensity.

Normal appearance of the cruciate/collateral ligaments.

Case Discussion

The clinical presentation and the MRI features are most consistent with a quadriceps fat pad impingement syndrome.
