Presentation

Patient experienced a sharp pain above the right knee after physical strain. Later, swelling of the knee developed, as well as reduced knee motion.

A rupture of the deep fibers of quadriceps muscle is shown, with preserved muscle tendon and a large hematoma in suprapatellar recessus. In distal part a hyperechoic collection is found, which on closer inspection represents a fat-fluid level. Due to it being indicative of the fracture, an X-ray examination was indicated.

Images acquired in upright and supine position show a fracture of the medial aspect of tibial plateau, with a free intraarticular bone fragment. A fat-fluid level is best visualized in supine position.
