There is a lesion in the quadrigeminal cistern on the left, with increased signal on T1WI and T2WI, chemical shift artifact on T2 and with a rim of 'blooming' on SWI. The combination of typical location and signal pattern is indicative of a quadrigeminal cistern lipoma.
Case Discussion
Intracranial lipomas are rare; they can be found near the midline and quadrigeminal cistern is one of their more frequent locations. They are usually discovered incidentally (like in this case) or can present with headache or epilepsy.
