MRI demonstrates a mass lesion at the left side of the quadrigeminal cistern measuring 12 x 15 x 17 mm, appears hyperintense on T1 and T2 weighted images, and shows a low signal on fat-suppressed images and blooming artifact in the T2* GRE sequence.

There is a minimal mass effect on the cerebellum and midbrain without hydrocephalus.

Case Discussion

Intracranial lipomas are rare tumors, with an incidence of 0.1% of all intracranial tumors, intracranial lipomas usually occur in midline areas, pericallosal (45%). quadrigeminal cistern (25%), suprasellar cistern (10%), cerebellopontine angle (10%), sylvian fissure (5%), choroid plexus lipoma (rare). Approximately 50% are associated with brain malformations (corpus callosum malformation and vascular abnormalities).

Intracranial lipomas are usually asymptomatic and incidentally detected and rarely require surgery.
