Age: 40 years
Gender: Female

Lesion of the quadrigeminal cistern, measuring approximately 13 x 7 x 12 mm, with high signal on T1W and T2W, low signal on FLAIR and GRE, no diffusion restriction, well-defined margins, with peripheral low signal on T1W and GRE (chemical shift artifact), and no significant mass effect. The remaining brain parenchyma is within normal limits.

Case Discussion

The MRI findings are characteristic of a lipoma in the quadrigeminal cistern. In the absence of fat-saturated sequences, the presence of chemical shift artifact on GRE will be helpful in confirming that the lesion contains fat. This is a case of incidental finding, with no clinical manifestations, so no further intervention is required.
