A triangular radiolucent area is observed along the anterior humeral line: finding compatible with the elevation of the anterior elbow fat pad secondary to joint effusion (sail sign).
No clear fracture is demonstrated.

CT scan reveals a compound, intra-articular, longitudinal course fracture of the radial head.
Case Discussion
The sail sign in the traumatic context raises the suspicion of a radial head fracture.
In the current case, a retrospective analysis of the X-ray reveals an extremely subtle radiolucent line matching the much more obvious fracture evident on the CT scan.
