Presentation
Postoperative day 1 (POD-1) following complex midline incisional hernia repair, the patient experienced acute abdominal pain and a large volume of fresh blood output from the drain.

A large midline abdominal wall heterogenous hematoma can be appreciated.
Case Discussion
The patient was taken to operating theater following clinical deterioration. The right rectus muscle was found to be oozing blood with a large volume of clotted blood collected in rectus sheath and hernia cavity. Washout was performed with good clinical outcome.
