Renal cell carcinoma - incidental on chest CT

Diagnosis probable

Presentation

Chronic cough.

Patient Data

Age: 80 years
Gender: Male

Large heterogeneous enhancing left adrenal mass. Enhancing mass appearing to involve the right kidney, extending along the presumed course of the left renal vein with heterogeneous enhancement of the inferior vena cava. 

No focal pulmonary lesion identified. 

Non-specific arterial enhancing lesion in the right posterior liver. 

Subsequent CT abdomen/pelvis confirms a right renal mass with extension into the left renal vein and inferior vena cava. The left adrenal mass is again demonstrated. 

Case Discussion

This is a gross example of always reviewing the check areas. While the abnormality is obvious in this case, if you don't look, you won't see. The upper abdominal findings are presumed to be metastatic renal cell carcinoma
