Renal cyst - Bosniak class IV

Presentation

Work up for abdominal pain.

Patient Data

Age: 85 years
Gender: Male

A 50×45 mm cystic lesion with thick enhancing irregular wall and internal septations is noted at the lateral aspect of the right kidney. In addition, an 8 mm enhancing mural node is observed at the posteromedial aspect (Bosniak class IV).

Additionally, several subcentimeter simple cortical cysts in both kidneys. 

In imaged portions of the lower thorax, several pulmonary nodules are seen less than 15 mm. A few adjacent subcarinal enlarged lymph nodes are observed.

The prostate gland is enlarged and a left inguinal hernia is incidentally noted. 

Case Discussion

The patient underwent a right nephrectomy and histopathology evaluation confirmed renal cell carcinoma.
