Renal replacement lipomatosis

Left renal parenchymal destruction with multiple calculi and renal replacement lipomatosis. A track is seen extending from residual left renal tissue extending inferiorly to left psoas forming a collection. Few dense foci noted in the collection suggestive of calculus migration through the track.
Case Discussion
Renal replacement lipomatosis is a rare condition characterized by fatty tissue proliferation in renal sinus and perinephric space with marked destruction/atrophy of renal parenchyma (due to chronic inflammation).
