Retinoblastoma

Diagnosis probable

Presentation

Right eye swelling.

Patient Data

Age: 4 years
Gender: Male
mri

Right orbital, posterolateral, retinal-based intraocular mass lesion with heterogeneous intermediate T1 signal, hypointense T2 signal, and diffusion restriction. There is a post-contrast enhancement with areas of very low signal non-enhancement likely representing intralesional calcification.

Associated focal posterolateral globe bulge (staphyloma).

There is no extraocular extension.

Diffuse right-sided vitreous intermediate to high signal T1 signal is suggestive of an associated vitreous hemorrhage.

Normal thickness, signal characteristic and enhancement pattern of right optic nerve as well as the extraocular muscles.

Normal left orbit.

Features are suggestive of globe confined unilateral right retinoblastoma, with associated vitreous hemorrhage .

Case Discussion

Retinoblastoma is the most common ocular malignancy in childhood, which is lethal if left untreated or diagnosed late.

The patient is transferred for preoperative chemotherapy with a further plan of transferring to a dedicated retinoblastoma treatment center.
