Presentation

Decreased hearing on left side.

Patient Data

Age: 30 years

Area of lucencies (demineralization) is seen around the bilateral cochlea, fissula ante fenestrum, foot plate of stapes and around bilateral semicircular canals, more pronounced on the left side.

Case Discussion

Otosclerosis is a misnomer, preferred term is otospongiosis due to lucent bony changes. Otospongiosis most commonly presents with hearing loss, most often conductive, but can also be sensorineural or mixed, and is frequently bilateral.

In this case, both fenestral and retrofenestral involvement is seen. Symons and Fanning's CT grading system1 shows this is a Grade 3 type.
