Abdominal distension - poor windowing on ultrasound

Age: 80 years
Gender: Female

A massive retroperitoneal solid expansive lesion fills the abdominal-pelvic cavity by displacing and encasing both retro- and intraperitoneal organs and compressing the pelvic structures. The mass demonstrates inhomogeneous attenuation, with evidence of varying amounts of fat and soft tissue-enhancing components.

Right moderate hydronephrosis, with the kidney displaced in the iliac fossa and the ureter, stretched and displaced contralaterally.

Pathology report (from italian):

[...] diagnosis: mesenchymal tissue sections with immature adipocytes, focal myxoid areas and moderate to severe nuclear atypia. Finding consistent with liposarcoma.

Case Discussion

Liposarcoma is one of the most common retroperitoneal masses. The finding of attenuation values typical of fat and soft tissue supports the diagnosis.
