Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Large right abdominal mass of complex echotexture on ultrasound.

Patient Data

Age: 5 years
Gender: Female

Large lobulated right suprarenal mass measuring 125 x 114 x 85 mm of complex density, composed of a large central fatty component with calcification/bone and cystic component mainly of peripheral location. No enhancement is seen following IV contrast. A mass effect is noted on the liver, duodenopancreatic region as well as the right kidney which is displaced inferiorly.

Case Discussion

CT features of a complex right suprarenal mass composed of fatty and cystic components with central calcification/bone highly suggestive of retroperitoneal teratoma, probably of adrenal origin.

Retroperitoneal teratomas are considered rare tumors in pediatric population, less frequent than neuroblastoma and Wilms tumor 1,2. The adrenal location is very rare and account for around 0.7% of all primary adrenal tumors.
