Presentation

Abdominal pain and fullness in the right side of the abdomen; headaches and palpitations.

Patient Data

Age: 50 years
Gender: Male

A large retroperitoneal cystic lesion with enhancing septa, solid component and fluid-fluid level in the lower pole of it was seen on the right side.

Case Discussion

The patient went to the operating room with the impression of a retroperitoneal hydatid cyst because the surgeon did not consult with an expert radiologist. During surgery, severe hypertension developed when the surgeon touched the lesion, retroperitoneal paraganglioma was suggested which confirmed on pathology.
