Reversal sign of diffuse hepatic fatty infiltration

Case contributed by Dr Lakshmi Srinivasan

Diffuse hypoattenuation of the hepatic parenchyma due to hepatic steatosis. The hepatic vessels appear hyperattenuating on this non-contrast study, but they are actually normal. This is an apparent 'reversal' of the normal attenuation relationship between the hepatic parenchyma and the hepatic vessels.

Elliptical ROI Hounsfield units of the liver measured -1.5 HU.

Note also, the marked pancreatic atrophy in this patient with cystic fibrosis.
