Posterior labroligamentous avulsion in the 6-9 o'clock position with a medially stripped intact periosteum.
There is also a reverse Hill-Sachs lesion of the anterior side of the humeral head.

Case Discussion
The Perthes lesion is a variation of the Bankart lesion, where the scapular periosteum is lifted and stripped medially with the detached labrum. The labrum may appear to be in a normal position at surgery but is still unstable.
