Presentation

Glenohumeral osteoarthritis with B2 glenoid (Walch et al) that is biconcave with posterior erosion and posterior humeral head subluxation.

Case Discussion

In the treatment of a glenoid with severe posterior erosion (i.e. Walch type B2), eccentric reaming can cause excessive medialization with reduction in glenoid size and de-tensioning of the rotator cuff. The augmented glenoid implants in shoulder artroplasthy, posteriorly, is a good solution for the restoration of the correct joint line. Pre-operative planning with specific software that is able to virtually implant the prosthesis and obtain patient-specific guides. Glenoid augments preserve more native bone stock and increasing glenoid lateralization with increasing the baseplate support and less glenoid reaming. Posteriorly augmented glenoid components can correct retroversion and avoid joint line medialization. The joint becomes a biomechanically stable structure.
