Reversed palmaris longus muscle

Presentation

Case of right wrist ulnar side pain. An incidental finding is present.

Patient Data

Age: 30 years
Gender: Male
ultrasound

The right wrist and distal forearm reveal a muscle echopattern structure which shows continuity with a tendon in the mid-proximal forearm. At the level of the wrist, it is outside the carpal tunnel and ulnar to the flexor carpi radialis tendon.

The contralateral side shows a normal palmaris longus tendon at the wrist.

Case Discussion

The case shows a reversed palmaris longus muscle. The muscle belly is located in the distal forearm and its tendon in the proximal-mid forearm.

The most common variation of the palmaris longus is an absent muscle1. Other variations are reversed, duplicated, bifid, or hypertrophied palmaris longus muscles1.
