Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Right paraspinal mass.

Patient Data

Age: 2 years
Gender: Female
mri

The MRI sequences demonstrate a large well-circumscribed multilobulated right paraspinal soft tissue mass centered on the erector spinae muscle, of intermediate signal intensity on T1WI, inhomogeneous high signal intensity onT2WI with intense and heterogeneous enhancement following IV contrast. A small area of necrosis is seen within the right lateral aspect of the mass. No intraspinal or endothoracic extension is seen.

Moderate hepatosplenomegaly with no other signs of Beckwith-Wiedemann syndrome.

Case Discussion

The MRI features are highly suggestive of rhabdomyosarcoma, and the mass was histopathologically proven as an embryonal subtype.
