Rheumatoid arthritis

Case contributed by Dr David Cuete

Presentation

Joint pain.

Patient Data

Age: 26
Gender: Female
X-ray

The radiograph shows carpal collapse and osteopenia.

CT

CT left wrist

CT shows carpal collapse, decrease of joint spaces and erosions at the distal radius, ulna and the carpal bones.

Case Discussion

The detection of erosions is important because they are a reflection of the aggressiveness of rheumatoid arthritis. CT is acquiring a recent role in this objective since it is the most sensitive and accurate technique to assess bone erosions.

The CT demonstrates the proper cortical discontinuity of the erosions and can differentiate of a geode. In advanced stages, inactive, a sclerotic rim surrounds the erosions.
