Conventional MRI images demonstrate subtle signal abnormalities of the right temporoparietal regions. DWI and ADC map show restricted diffusion with high SI on DWI and low SI on ADC map corresponding to the infarcted area.

Case Discussion

The middle cerebral artery is one of three major paired branches that supply the brain and it covers almost all of the lateral surface of the brain. It is subdivided into four segments M1, M2, M3 and M4.

The patient with symptoms on the left side demonstrates abnormal subtle high FLAIR signal of the right tempro-parietal lobe. No evidence of signal change on T1WI...MRI findings are keeping with late hyperacute right middle cerebral artery infarction.
