Right middle lobe syndrome

Presentation

Colorectal carcinoma, chest CT done for metastatic work up.

Patient Data

Age: 45 years
Gender: Male

Chest CT in the lung window shows the complete collapse of the right middle lobe with dilated bronchograms in the atelectatic lobe. The right middle lobe bronchus appears patent, with no endobronchial lesion to suggest metastasis. Few pleural-based lung nodules are seen. Bilateral mild emphysematous changes are also noted.

Case Discussion

The patient had a chest radiograph several months prior to this CT (not shown) which demonstrated a similar finding (right middle lobe collapse). This is in keeping with chronic, non-obstructive right middle lobe collapse, also termed right middle lobe syndrome.
