Right-sided aortic arch with aberrant left subclavian artery and kommerell diverticulum, which is dilated compressing the esophagus. Dilated tortuous descending thoracic aorta.

Scans through the upper abdomen revealed:

  • a large obstructing stone at right kidney casting its pelvis and extending to middle and lower calyces exerting hydrocalycosis. Multiple lower calyceal non-obstructing stones are also noted
  • small left kidney with atrophic parenchyma and lower calyceal non-obstructing stone. It is as shows a rounded mid-zone homogeneous soft tissue density lesion that is worth a dedicated multiphasic kidney CT
  • gallbladder contracted over few hyperdense stones

Case Discussion

Right-sided aortic arch is a type of aortic arch variant characterized by the aortic arch coursing to the right of the trachea. In this case, it is associated with an aberrant left subclavian artery and kommerell diverticulum which is dilated compressing the esophagus.
