Presentation

Difficulty breathing and expanding neck hematoma post intracardiac electrophysiology study (EPS).

Patient Data

Age: 65 years
Gender: Female

ct

CT neck shows a large hematoma in the right supraclavicular region. The superficial component thickens the strap muscles and the right sternocleidomastoid muscle.

The deep component extends the length of the carotid sheath to the level of skull base causing displacement of right parapharyngeal fat pad, distortion of oropharynx and mild leftward tracheal displacement without compression.

No active contrast extravasation.

Case Discussion

The patient's enlarging neck hematoma was associated with a hemoglobin drop of 148 to 120 mg/dL.
