Right upper lobe pneumonia

Presentation

Cough and fever.

Patient Data

Age: 60
Gender: Male
x-ray

Large rounded region of consolidation with the right mid zone which is confirmed to be within the right upper lobe on the lateral image. Exact location of the horizontal fissure is unknown but this may be bulging the fissure. Air bronchograms are well seen on the lateral image. 

Case Discussion

Although this is likely to be a simple case of pneumonia given the presenting history, it is always important to recommend a follow-up chest x-ray following antibiotic therapy to confirm resolution in a patient this age. In younger patients, where the risk of lung cancer is very low, a follow-up chest x-ray is usually not necessary and should not be routinely recommended. The rounded shape and bulging fissure in this case may be further reason in addition to the patient's age to recommended follow-up.
