Persistent left shoulder pain for three months.
Patient Data
STIR images through the infraspinatus muscle tendon insertion demonstrate a fluid-filled defect at the articular surface, extending through greater than 50% of the cuff thickness. The tear involves the tendon-bone interface at the articular surface and extends slightly into the distal substance of the supraspinatus tendon. The bursal-sided fibres remain intact and attached to the rotator cuff footprint. There's also an insertional partial tear of the subscapularis tendon on the articular side. The long head of the biceps tendon is normal
Case Discussion
Rim rent tear or PASTA lesions appear as linear fluid signal defects at the articular surface, near the supraspinatus-infraspinatus tendon-bone interface in young patients or athletes. The margins of the tear are often sharply defined and the tear. In the elderly population, articular-sided partial-thickness tears may also occur as a degenerative process. Many rim-rent tears will progress to full-thickness tears if not diagnosed and not treated.
