Diagnosis probable

Presentation

Prolactin level at 130.2 ng/mL.

Patient Data

Age: 40 years
Gender: Male

Normal dimensions of the pituitary gland with small (7 x 6 x 4 mm) hypoenhancing nodule of the right aspect of the pituitary gland around the inferomedial aspect of the supraclinoid ICA. The infundibulum is centered with normal thickness and enhancement. Normal appearance of the optic chiasma and cavernous sinuses.

Small ring-shaped nodules along the wall of the left lateral ventricle, isointense to the cortical grey matter on T1, T2 and high signal FLAIR with no enhancement on post-contrast sequences.

Case Discussion

MRI features suggestive of ring-shaped lateral ventricular nodules (RSLVNs), incidental findings in a patient with pituitary microadenoma.

Ring-shaped lateral ventricular nodules (RSLVNs) are small benign nodular ring-shaped lesions attached to the ependyma of the roof and body of the lateral ventricles. Usually asymptomatic and discovered incidentally during CT or MRI exam. Thee precise nature of these nodules remains unknown due to the insufficient histopathological data.

The main differential diagnosis includes:

  • subependymal heterotopia: usually in patients with epilepsy

  • subependymal nodules: patients with tuberous sclerosis
