Presentation
Left shoulder recurrent pain for last 3 years. Increased pain for the last 2 weeks. No history of trauma.
Patient Data

The biceps long head tendon is intact, normal in position without tenosynovitis. There are few small (<5 mm) calcification foci in the subscapularis tendon. There is a large calcification focus (about 20 x 14 x 6 mm) involving supraspinatus and infraspinatus tendons. The rotator cuff is intact.
Local compression shows movements of the calcium in the supraspinatus-infraspinatus tendons and extra-tendinous extension to the sub-bursal location.
There is no subacromial-subdeltoid bursal effusion/ glenohumeral joint effusion.
Case Discussion
The case shows calcific tendinosis involving rotator cuff and sub-bursal leak of the calcium.
