Round pneumonia

Case contributed by Yusra Sheikh
Diagnosis certain

Presentation

Presented to the emergency department with vomiting, diarrhea and shortness of breath.

Patient Data

Age: 7 years
Gender: Male
x-ray

There is a round opacity in the right perihilar mid zone region with somewhat irregular margin. The right cardiac border is not obscured. A further area of airspace opacification is seen superior to the mass like round opacity. There is no pleural effusion. Heart size and cardiomediastinal contours are normal. Oxygen tubing mask noted.

x-ray

Follow up chest radiograph 6 weeks after initial presentation. Both lungs are clear with complete resolution of previously demonstrated areas of air space opacification.

Case Discussion

Findings in this case are most suggestive of a round pneumonia. A similar appearance of the initial radiograph in an adult patient may be worrisome and warrant further imaging.
