Presentation
Fever and cough for 3 days.
Patient Data

A round opacity in the left lower lobe posterior to the heart.
Lungs otherwise clear.

Air space consolidation containing air bronchograms in the posterior segment of the left lower lobe with surrounding ground glass opacification indicating round pneumonia.
Case Discussion
Round pneumonia is usually only seen in paediatric patients in whom the spread of infection is limited by incomplete development of interalveolar connections.
The retrocardiac lung is one of the areas where lesions can be overlooked due to superimposition of structures. Other important review areas are the lung apices, hilar zones and the zone below the dome of the diaphragm.
