Diagnosis almost certain

Presentation

End stage renal disease.

Patient Data

Age: 45 years
Gender: Male
ct

The spleen is enlarged and its cephalocaudal height measured 170mm.
Both kidneys are small with atrophic parenchymal changes. A few subcentimeter simple cortical cysts are seen at kidneys. 
Transplanted kidney with relatively atrophic parenchyma and a few small simple cortical cysts are seen at right iliac fossa. Mild hydronephrosis is present without obvious stone or mass.
Diffuse but mild urinary bladder wall thickening is present.
Mild degenerative changes as osteophytosis are seen at the lumbar spine.
There are also prominent endplate densities at multiple contiguous vertebral levels.
